You have a new choice in your health care. The Illinois Department of Healthcare and Family Services (HFS) offers a new way for you and your family to get quality health care. If you live in Adams, Brown, Cook, Henry, Jackson, Kane, Lee, Madison, McHenry, Mercer, Perry, Pike, Randolph, Rock Island, Scott, St. Clair, Washington or Williamson county and have an HFS or All Kids medical card, you can probably sign up. If you do not live in one of these counties, visit the Illinois Health Connect website to learn about your health care choices.

If you are an older adult, age 60 and over, or an adult with disabilities, age 19 to 59, and live in suburban Cook (Non 606 zip codes), DuPage, Kane, Kankakee, Lake or Will Counties, you are a member of the Integrated Care Program. Visit the Integrated Care Program website at www.illinoiscebicp.com.

When you enroll, you pick a health plan and primary care provider (PCP) for your medical home. These are the health plans you may pick from if you live in Cook County:

Family Health Network (a managed care organization that is like an HMO)
Harmony Health Plan (an HMO)
Illinois Health Connect (a Primary Care Case Management Program)
Meridian Health Plan (an HMO)

If you live in Jackson, Kane, Madison, Perry, Randolph, St. Clair, Washington or Williamson county, you may pick from these health plans:

Illinois Health Connect (a NEW Primary Care Case Management Program)
Harmony Health Plan (an HMO)

If you live in Adams, Brown, Henry, Lee, McHenry, Mercer, Pike, Rock Island or Scott county, you may pick from these health plans:

What is a Medical Home?

Your medical home is the doctor's office or clinic where you go to see your PCP. Having a medical home means:

All your medical records are kept there.
Your PCP gets to know you well.
You get better health care because your PCP knows all of your health care needs.
You and your family get the top quality health care you need to stay healthy.

Even if you are healthy and never get sick, it is good to have a medical home.

What is a Primary Care Provider (PCP)?

Your Primary Care Provider (PCP) is your family doctor, nurse of other health care provider at your medical home who takes care of you and your family. Your doctor will:

Take care of you when you are sick or need medical attention.
Give your children regular check-ups and immunizations (shots) to stay well.
Help you manage diseases and other conditions like diabetes, high blood pressure, and asthma.
Send you to specialists or other health care providers when you need to go. This is called a referral.
Answer questions you have about your health care.
Give you the information you need to stay healthy.
Work with you to get the health care you need.

MAIL: Fill out the enrollment form in the packet you received in the mail. Send the form back to the Illinois Client Enrollment Broker in the envelope provided.
