You have a new choice in your health care. The Illinois Department of Healthcare and Family Services (HFS) offers a new way for you and your family to get quality health care. If you live in Adams, Brown, Cook, Henry, Jackson, Kane, Lee, Madison, McHenry, Mercer, Perry, Pike, Randolph, Rock Island, Scott, St. Clair, Washington or Williamson county and have an HFS or All Kids medical card, you can probably sign up. If you do not live in one of these counties, visit the Illinois Health Connect website to learn about your health care choices.

If you are an older adult, age 60 and over, or an adult with disabilities, age 19 to 59, and live in suburban Cook (Non 606 zip codes), DuPage, Kane, Kankakee, Lake or Will Counties, you are a member of the Integrated Care Program. What is a Medical Home?
