How do I make the best choice?
A provider is your PCP. Your PCP can be a doctor, nurse or other health care professional that you go to for checkups or when you have a health problem.
In Cook County:
Think about where you and your family get your health care now. Do you want to stay with your current doctor or health plan? Do you want to make a change? Ask your doctor or the doctor you want if they are part of Illinois Health Connect, Family Health Network, or Harmony Health Plan or call the Illinois Client Enrollment Broker Helpline at 1-877-912-8880 (TTY: 1-866-565-8576) to find out.
In Jackson, Madison, Perry, Randolph, St. Clair, Washington, and Williamson Counties:
Think about where you and your family get your health care now. Do you want to stay with your current doctor or health plan? Do you want to make a change? - If they are in your health plan, you can pick them as your PCP. You do not have to pick the same PCP for all members of your family.
- If they are not in your health plan, call the ICEB at 1-877-912-8880 (TTY: 1-866-565-8576). We will help you find a PCP.

When choosing a provider as your PCP, think about.
- How far am I willing to travel to see a doctor?
- Is there a doctor I already like?
- Do I (or a family member) have any special health needs?
- Do I need a doctor who speaks a certain language?
- Is the provider I want for my kids in the All Kids program?
